TERMS AND CONDITIONS:

By participating in any soccer training sessions provided by Krzysik Soccer Training LLC, you acknowledge and agree to the following terms and conditions:

• Assumption of Risk: Soccer training involves physical activity and inherent risks, including but not limited to falls, collisions, and other potential injuries. By participating, you voluntarily assume all risks associated with the activity.

• Release of Liability: Krzysik Soccer Training LLC, its trainers, coaches, and staff shall not be held liable for any injuries, damages, losses, or expenses that may occur during or as a result of your participation in the training sessions.

• Responsibility: Participants are responsible for ensuring that they are in suitable physical condition to safely participate in the soccer training sessions. It is advised to consult with a medical professional if you have any health concerns before engaging in the activities.

• Supervision: Participants under the age of 18 must be under the direct supervision of a parent or legal guardian during the training sessions.

• Personal Belongings: Krzysik Soccer Training LLC is not responsible for any loss, theft, or damage to personal belongings brought to the training premises.

• Conduct: All participants are expected to adhere to appropriate behavior and respect the instructions of the trainers and staff. Any disruptive or harmful behavior may result in removal from the training sessions.

• Media Consent: By participating in the training, you grant Krzysik Soccer Training LLC the right to use photographs, videos, or other media for promotional purposes without any compensation.

• Changes to Schedule: Krzysik Soccer Training LLC reserves the right to modify or cancel training sessions, schedules, or locations due to unforeseen circumstances. Reasonable efforts will be made to notify participants in advance.

• Governing Law: These terms and conditions shall be governed by and construed in accordance with the laws of the United States of America, Arizona, without regard to its conflict of laws principles.

USER AGREEMENT:

I certify that my child has no known medical or other conditions that could interfere with his/her participation in Krzysik Soccer Training LLC sessions. It must be stated to us, and is the sole responsibility of the parent, nanny, caregiver or friend who brings the client to class.

I also understand and accept that the practice of soccer, both indoor and outdoor, involves certain risks of mental, emotional or physical injury.

Therefore, I, individually and on behalf of my child, hereby release, discharge, and hold harmless Krzysik Soccer Training LLC and each of its respective parent, subsidiary, and affiliated companies, and each of their respective officers, directors, agents, representatives, employees, successors, assignees, and licensees (hereinafter, “the Released Parties”) from any and all claims, actions, damages, losses, liabilities, costs and expenses of any kind whatsoever, including but not limited to any claims of negligence, arising out of, resulting from, by reason of, or in connection with my child’s participation in any Krzysik Soccer Training LLC program or activity.

I agree that I will not bring or be a party to any legal action or claim against the Released Parties, or any of them, based upon or arising out of my child’s participation in any Krzysik Soccer Training LLC program or activity on any legal theory whatsoever (including without limitation personal injury, negligence, rights of privacy and publicity, or defamation).

I understand and agree that Krzysik Soccer Training LLC has the right to terminate the enrollment of any child from Krzysik Soccer Training if it, in its sole discretion, determines that the continued attendance of the child in the program is not in the best interest of the child nor the program. I agree to supervise or designate a responsible adult to supervise my child prior to and immediately after his/her Krzysik Soccer Training LLC activities.

I understand and agree that in case of an allergic reaction, I am solely responsible for attending to and treating my child.

Krzysik Soccer Training LLC clients (parent of legal guardian) must ALL sign our legal waiver and parental consent form before they can step onto a Krzysik Soccer Training LLC designated area or field, this includes any and all Free Session(s). No exceptions. 

I agree that I the parent/guardian/ family member or friend who chooses to use Krzysik Soccer Training LLC equipment prior to, during or after class, is considered theft by Krzysik Soccer Training LLC and Krzysik Soccer Training LLC takes no legal responsibility for ANY potential injury both physical or mental to any member (family or friend) taking part. 

I agree that Krzysik Soccer Training LLC equipment is only to be used by Krzysik Soccer Training LLC staff and clients(children) who have been signed into class via our legal waiver. I agree that Krzysik Soccer Training LLC staff are the only people who can break down and build, move or carry our fields equipment. 

I agree and understand that Krzysik Soccer Training LLC does NOT guarantee a certain coach for your child or for your class and Krzysik Soccer Training LLC reserves the right to allocate its staff. 

I agree that when my sessions reach an end that I must renew to be in the class the following week or designated time. 

By engaging in any soccer training sessions offered by Krzysik Soccer Training LLC, you acknowledge that you have read, understood, and agree to abide by these terms and conditions. If you do not agree with any part of these terms, please refrain from participating in the training sessions.

CANCELLATION POLICY:

At Krzysik Soccer Training LLC, we are committed to providing top-quality training sessions and ensuring the best experience for our clients. To maintain the efficiency and fairness of our operations, we have implemented the following cancellation policy:

1. Refunds and Credits: Please be aware that we do not offer refunds or credits for missed training sessions, regardless of the reason. All package one on one sessions (4,8,12) are final purchase. You can not purchase 12 sessions and only use 6 then ask for your money back for the remaining 6 sessions, as goes with our other packages. No credits or refunds on one-on-one package sessions or unused sessions in a package. Package sessions are valid for 90 days from date of purchase and there are no refunds for sessions not used in the 90 day period. After 90 days you must renew a new one on one sessions package.

2. Makeup Sessions for Unforeseen Family Circumstances: The only time we will offer makeup sessions is in the case of unforeseen family circumstances, such as family illness. If you encounter such a situation, please contact us promptly to discuss rescheduling options.

3. Weather-Related Cancellations: On rare occasions, extreme weather conditions or other unforeseeable circumstances may necessitate the cancellation of training sessions. In these cases, we will make every effort to reschedule the session for a later date.

4. Changes to Training Schedule: Krzysik Soccer Training LLC reserves the right to modify the training schedule, including dates, times, and locations, if necessary. We will communicate any such changes to participants as early as possible and make reasonable efforts to accommodate any conflicts arising from the schedule changes.

By participating in Krzysik Soccer Training LLC sessions, you acknowledge and accept the terms of this cancellation policy. We appreciate your understanding and cooperation as we work to provide you with the best soccer training experience possible. If you have any questions or need to request a makeup session due to an unforeseen family circumstance, please feel free to reach out to us. Your child’s success in our soccer training program is important to us, and we are here to assist you.

KST Elite Terms and Conditions

By subscribing to the KST Elite program, you agree to the following terms and conditions:

1. Membership Overview: KST Elite offers a monthly subscription that includes unlimited one-on-one training sessions with Coach Jon Krzysik. This program is specifically designed for one player only, providing personalized attention and coaching to enhance individual skills and performance.

2. Payment Terms: The monthly subscription fee for KST Elite is $415, billed at the beginning of each billing cycle. Payments are non-refundable, and you acknowledge that your subscription will automatically renew each month until canceled.

3. Minimum Commitment: A minimum commitment of two months is required for all new members. During this initial period, you may not cancel your subscription. This commitment allows for continuity in training and the opportunity to see significant improvements.

4. Cancellation Policy: After the initial two-month commitment, you may cancel your subscription at any time. To initiate a cancellation, you must provide written notice via email or through our contact form. Cancellations will take effect at the end of the current billing cycle, and no further charges will be incurred.

5. Session Scheduling and Flexibility: Members enjoy priority booking for training sessions. All sessions must be scheduled in advance through our booking system. While we encourage regular training, we understand that conflicts may arise. Members can reschedule any training sessions as needed, based on availability.

6. Cancellation of Scheduled Sessions: If you need to cancel a scheduled training session, please provide at least 12 hours’ notice. This will allow us to offer that time slot to other members. Canceled sessions can be easily rescheduled based on your availability and our scheduling options.

7. Exclusive KST Nike Training T-Shirt: Upon subscribing, members will receive an exclusive KST Nike training t-shirt. Members will have the opportunity to select their preferred t-shirt size during the registration process.

8. Modifications to Terms: KST Soccer Training reserves the right to modify the terms of the KST Elite program at any time. Members will be notified of any significant changes to the terms and conditions.

9. Liability Waiver: By participating in training sessions, members acknowledge that they are participating at their own risk. KST Soccer Training will not be held liable for any injuries or accidents that may occur during training.
